y <br /> off- <br /> MEMORANDUM OF AGREEMENT BETWEEN <br /> THE UNIVERSITY OF NORTH CAROLINA AT CHAPEL HILL <br /> CHAPEL HILL, NORTH CAROLINA <br /> for the <br /> DEPARTMENT OF DENTAL ECOLOGY <br /> in the <br /> SCHOOL OF DENTISTRY <br /> and <br /> Orange County Health Department <br /> for the <br /> Smart Start Grant <br /> I. PURPOSE <br /> This document is a Memorandum of Agreement between The University of North Carolina <br /> at Chapel Hill, for its Department of Dental Ecology in the School of Dentistry, hereafter <br /> referred to as the "University," and Orange County Health Department, hereafter referred <br /> to as the "OCHD", for the purpose of providing a Coordinator for its Smart Start Grant <br /> awarded by the Orange County Partnership for Young Children, hereafter referred to as <br /> "OCPYC" <br /> II. EFFECTIVE DATES <br /> This agreement shall be effective as of April 1, 1994 and shall remain in effect until June 30, <br /> 1994 unless terminated as set forth below. Either party may request a change in the terms <br /> of this agreement. Any agreed to change to this agreement must be approved by both parties' <br /> written endorsement thereto. Either party may terminate this agreement by giving the other <br /> party 60 days written notice in advance of the desired termination date. <br /> III. GENERAL AGREEMENT <br /> During the performance of this agreement, both parties agree as follows: <br /> A. The OCHD hereby agrees with the University that, in its educational and/or <br /> employment practices, said contracting party will not discriminate against any person <br /> because of race, color, religion, sex, national origin, age, or disability. <br /> B. The OCHD will not be responsible for damages arising from personal injury to <br /> students of the University while they are present at the OCHD unless personal injury <br /> was caused by the negligence of the OCHD, its employes, or agents. <br /> C. In the event the student is exposed to blood or body fluids, the OCHD will be <br /> responsible for providing an initial response, according to the Institution's Blood- <br /> . Borne Pathogens Standard. The student will be required to return to University of <br /> North Carolina Student Health Services for post-incident testing and counseling if <br /> feasible. In the event the student is unable to do so in a timely manner, the OCHD <br /> will provide access to testing and counseling for the student. The student will be <br /> responsible for the payment of services, but will be able to request reimbursement <br /> from the University, <br />
