Docusign Envelope ID : 9CB7D959-6861 -412D-BC22-4F1F2FA96832 � VA <br /> � \'^ `" �✓ � - �� � g �� I Os� <br /> NORTH CAROLINA <br /> CONTRACT AMENDMENT <br /> ORANGE COUNTY <br /> THIS CONTRACT AMENDMENT ("Amendment") is made and entered into this 22nd day of October, 2025 by and <br /> between ORANGE COUNTY (hereinafter referred to as "County") and Freedom House Recovery Center, Inc. <br /> (hereinafter referred to as "Provider") , <br /> WITNESSETH : <br /> THAT WHEREAS , the County and Provider entered into a contract dated July 1 , 2024, (hereinafter the "Original <br /> Agreement") , for the provision of services for support programs that serve persons with Opioid Use Disorder (OUD) ; and <br /> WHEREAS , the County and Provider desire to amend the Original Agreement while keeping in effect all terms and <br /> conditions of the Original Agreement not inconsistent with the terms and conditions set forth below. <br /> NOW THEREFORE, for and in consideration of the mutual covenants and agreements made herein, the parties agree to <br /> amend the Original Agreement as follows : <br /> 1 . In order to ensure the completion of the Services identified in the term of the Original Agreement is amended <br /> to reflect an end date by which all Services shall be completed of December 31 , 2025 . <br /> 2 . Article 3 . , Section a. is amended to reflect additional invoice dates of October 31 , 2025 for service period July <br /> 1 , 2025 through September 30 , 2025 , and if funding isn 't fully utilized by that time January 15, 2026 for service <br /> period October 1 , 2025 through December 31 , 2025 . Supporting documentation is due with the invoice . <br /> 3 . Article 3 . , Section a. is amended to reflect an additional reporting date of October 31 , 2025 for service period <br /> July 1 , 2025 through September 30, 2025 , and if funding isn ' t fully utilized by that time January 15 , 2026 for <br /> service period October 1 , 2025 through December 31 , 2025 . The narrative will also be required at the time of <br /> the final report . <br /> 4 . Except for the changes made herein, the Original Agreement shall remain in full force and effect to the extent <br /> it is not inconsistent with this Amendment.
