Revised 01/24 <br /> <br />NORTH CAROLINA <br />CONTRACT AMENDMENT <br />ORANGE COUNTY <br /> <br /> <br />THIS CONTRACT AMENDMENT (“Amendment”) is made and entered into this 22nd day of October, 2025 by and <br />between ORANGE COUNTY (hereinafter referred to as “County”) and Freedom House Recovery Center, Inc. <br />(hereinafter referred to as “Provider”). <br /> <br /> <br />WITNESSETH: <br /> <br />THAT WHEREAS, the County and Provider entered into a contract dated July 1, 2024, (hereinafter the “Original <br />Agreement”), for the provision of services for support programs that serve persons with Opioid Use Disorder (OUD); and <br /> <br />WHEREAS, the County and Provider desire to amend the Original Agreement while keeping in effect all terms and <br />conditions of the Original Agreement not inconsistent with the terms and conditions set forth below. <br /> <br />NOW THEREFORE, for and in consideration of the mutual covenants and agreements made herein, the parties agree to <br />amend the Original Agreement as follows: <br /> <br />1. In order to ensure the completion of the Services identified in the term of the Original Agreement is amended <br />to reflect an end date by which all Services shall be completed of December 31, 2025. <br /> <br />2. Article 3., Section a. is amended to reflect additional invoice dates of October 31, 2025 for service period July <br />1, 2025 through September 30, 2025, and if funding isn’t fully utilized by that time January 15, 2026 for service <br />period October 1, 2025 through December 31, 2025. Supporting documentation is due with the invoice. <br /> <br />3. Article 3., Section a. is amended to reflect an additional reporting date of October 31, 2025 for service period <br />July 1, 2025 through September 30, 2025, and if funding isn’t fully utilized by that time January 15, 2026 for <br />service period October 1, 2025 through December 31, 2025. The narrative will also be required at the time of <br />the final report. <br /> <br />4. Except for the changes made herein, the Original Agreement shall remain in full force and effect to the extent <br />it is not inconsistent with this Amendment.
