No Resident Profile Yes No N/A Comments/Other Observations (please number comments) 1. Do the residents appear neat, clean and odor free? Yes 1. Everyone was dressed neatly. We arrived shortly before lunch so many residents were sitting near the dining room. 5a. Residents in Memory Care were sitting at tables waiting for lunch. We observed one staff member put a resident's plate of foord in front of him without any interaction. This resident had very limited ability to communicate and need mashed food. 5b. Senior or program staff had nametags, but not CNAs, cleaning staff, etc. 2.Did residents say they receive assistance with personal care activities? (i.e. brushing their teeth, combing their hair, inserting dentures or cleaning their eyeglasses) Yes 3. Did you see or hear residents being encouraged to participate in their care by staff members? Yes 4.Were residents interacting with staff, other residents & visitors? Yes 5.Did staff respond to or interact with residents who had difficulty communicating or making their needs known verbally? Did residents describe their living environment as homelike? Yes 8. The main dining room was very hot. Residents said that the airconditioning had been broken for several weeks. 10. In one of the recreation rooms on top of the piano there was a vase of water that had apparently had flowers. If a resident tried to reach or drink this it could have been dangerous. 12. New management changed the rules so that smoking is permitted only on the back porch, not the front. 9. Did you notice unpleasant odors? No 10.
