Please sign each copy. with <br />an original. signature. After the documents have been. signed by the appropriate <br />authorizing official, please retain one copy for your records and return the remaining four <br />copies to this office by overnight mail. along with a HUD Form 270 for the full amount <br />indicated below. The funding for this Amendment includes: <br />TOTAL AMOUNT OF: $53,500.00 <br />Case Processing: $18,000.00 10 cases @.$1800 <br />Cause (charged) Cases: $ 500.00 1 case @ $500. <br />Training: $18,000.00 <br />Outreach/Education: $ 2-,000.00 <br />SEE $0 :. <br />Administrative Cost $15,000.00 <br />In addition, we have enclosed a checklist with this letter.. Please complete the <br />checklist by placing your initials in the appropriate space. By placing your .initials in the <br />appropriate space, you certify that you have submitted the information to HUD, or have the <br />information in your possession to support: processing of the current voucher.
