Explanation of Services to be provided and to whom (include SIS Service Code): _ <br />The contractor will facilitate earl identification and treatment of children who are at risk <br />for Mental Health (social emotional) problems This will include establishing routine <br />• mental health screening for children ale birth through 5 who are receiving Child <br />Protective Services and providing family-focused mental health assessment for those <br />children identified by screening The Contractor is required to meet all coals and <br />outcomes listed in Attachment N. <br />C.
