<br />FINANCIAL CONFLICT OF INTEREST <br />The SUBCONTRACTOR certifies that it maintains an appropriate written, enforced policy on <br />conflict of interest that complies with the provisions of 42 CFR Part 50. <br />ACKNOWLEDGMENT OF FEDERAL FUNDING <br />The SUBCONTRACTOR agrees to comply with the requirements as follows: "When issuing <br />statements, press releases, requests for proposals, bid solicitations, or other documents describing <br />projects or programs funded in whole or in part with Federal money, all grantees receiving <br />Federal funds, including but not limited to State and local governments and recipients of Federal <br />research funds, shall clearly state (1) the percentage of the total costs of the program or project <br />which will be financed with Federal money, (2) the dollar amount of Federal funds for the <br />program or project, and (3) percentage and dollar amount of the total costs for the project or <br />program that will be financed by non-governmental sources. " <br />CIVIL RIGHTS AND EQUAL EMPLOYMENT OPPORTUNITY <br />The SUBCONTRACTOR shall comply with Title VI and VII of the Civil Rights Act of 1964, and <br />Executive Order 11246 and certifies that it has a valid Assurance of Compliance on file with the <br />HHS (Form HHS 690}. The SUBCONTRACTOR agrees to include in any contract entered into <br />pursuant to this Agreement, paragraphs (1) through (7), Part II, Subpart B, Section 202, Executive <br />Order 11246. <br />9 <br />
