07/31/08: LT Joseph contacted Cliff Cates by telephone. Advised him that <br />no information received from Chief Matthews and that the documents are <br />necessary to begin the proper assessment. He advised that he would attempt <br />to gather the documents that afternoon. No documents received. <br />08/04/08: LT Joseph receives a voice -mail message in his office from Chief <br />Matthews advising that he would have the documents. LT Joseph requests <br />documents be delivered no later than the first of the week (8/4 — 8/8). <br />08/05/08: LT Joseph receives an e -mail from Dan Seip asking if there were <br />any other documents needed to continue the assessment process. <br />08/05/08: LT Joseph replies to Dan Seip via e-mail informing him that no <br />documents had been received as requested. No documents requested <br />received. <br />08/05/08: OCRS roster and training documents delivered to ES headquarters <br />by Chief Matthews. Accepted by Lysa May. <br />08/15/08: LT Joseph a -mails Dan Seip to meet with him. <br />08/18/08: Dan Seip indicates he cannot meet. <br />08/29/08: Dan Seip asks if there is anything else to be received and how the <br />review is going. <br />
