<br />~~ Orange County Health Department <br />Fostgraduate Year One Level <br />July 1, 2000 <br />3 <br />Dentistry will provide attending dentist supervision to the resident providing services under this Agreement. _ <br />3. School of Dentistry will bill The Health Departrrrent at the close of each quarter far the salary/fringe of one <br />postgraduate year one resident. <br />4. The Cooperating Institution must provide HIV and HBV testing for the cni~mP patient In the event of a <br />bloodborne pathogen exposure. In the event the student is exposed to blood or body fluids, the <br />Cooperating Institution will be respansrble for providing an initial response, according to the Institution's <br />Blood-Borne Pathogens Standard. The student will be required to return to University of North Carolina <br />.Hospital's Occupational Health Service for post incident testing and counseling if feasible. In the event the <br />student is unable to do so in a timely manner, the Cooperating Institution will provide access to testing and <br />counseling for the student. The trainee will be responsible for the payment of services. UNC will <br />reimburse trainee for testing and counseling only but not for any emergency treatment or medication <br />administered. <br />S. The person whose services are to be provided pursuant to the Agreement is, for all purposes, an employee <br />of The University ofNorth Carolina Hospitals. <br />6. While the University of North Carolina Hospitals resident is on rotation at the Orange County Health <br />Department, their professional liability ;n~,~,~„~e coverage will be provided by the School of Dentistry <br />with coverage of at least $1 million, per incident, $3 million per year. <br />7. This Agreement shall run for a period of one year, from the 1st day of July, 2000 to the 30th day of June, <br />2001, and shall be renewable thereafter. <br />8. This Agreement or its renewaL9 maybe terminated at any time without penalty by either Party provided that <br />written notice of such termination is ftunished ~ the other parry at least 90 days prior to termination. In <br />the event of such termination any payment due shall be prorated to the date of termination. <br />9. In compliance with 42 U.S.C. 1395x (v)(1x1) and implementing regulations, The School of <br />Dentistry and the Health Department agree, until the expiration of four years after the services are <br />famished under this contract, to allow the Secretary of the Department of Health and Human Services and <br />the Compti+oper General access to this contract and to the books, docturrents and records of The School of <br />Dentistry and The Health Department necessary to verify the nature and extent of this contract.
