The following <br />documentation should be mailed to HUD at the time <br />of reporting the closure to HUD: <br />(a) A chronology of agency actions prior to the <br />withdrawal request; and <br />(b) A copy of the signed and dated withdrawal <br />request. <br />V. ADMINISTRATIVE CLOSIIRES - If over 20~ of the agency's total <br />caseload are closures as discussed in A through D of this <br />section, the agency is ineligible for SEE funds (reference <br />115.305(b)). <br />A. Withdrawals Without Resolution <br />1. Re irements for Acce tance. The withdrawal <br />request must: <br />(, a) Be in writing; <br />(b) Be signed and dated by the complainant ar an <br />authorized representative; <br />(c) Identify the respondent(s); <br />(d) Contain the HUD case number and the State or <br />local agency case number; <br />(e) Indicate the reason(s) why complainant wishes <br />to withdraw the charge (see also VI.3 of this <br />criteria); <br />(f) Contain a specific reference to the <br />withdrawal of the HUD complaint as well as <br />the agency's co<<lplaint; and <br />(-g) Indicate the complainant's awareness that the <br />withdrawal will result in the termi;iation of <br />proceedings involving the matter; and <br />(h) Contain a specific statement that there was <br />no coercion or fear of retaliation from any <br />person (not j ust the. respondent) . <br />
