<br />2 <br />MEMORANDUM OF AGREEMENT $ETWEEN <br />THE UNIVERSITY OF NORTH CAROLINA AT CHAPEL HII.L <br />CHAPEL HILL, NORTH CAROLINA <br />for the <br />DEPARTMENT OF DENTAL ECDLOGY <br />in the <br />SCHOOL OF DENTISTRY <br />and . <br />Orange County Health Department <br />L PURPOSE <br />Tlvs document is a Memorandum of Agreement between The University of North Carolina at Chapel Hill, <br />for its Department of Dental Ecology in the School of Dentistry, hereafter referred to as the "University," <br />and Orange County Health Department, hereafter referred to as the "OCHD", for the purpose of providing <br />a Children's Dental Care Coordinator. <br />IT. EFFECTIVE DATES <br />This agreement shall be e$ective as of April 1, 2000 and shall remain in effect until June 30, 2000 unless <br />ternunated as set forth below. Either party may request a change in the tern~.s of this agreement. Any <br />change to this agreement must be approved by both parties' written endorsement thereto. Either party may <br />terminate this agreement by giving the other party 30 days written notice in advance of the desired <br />termination date. <br />III. WITNESSETH <br />WHEREAS, The Health Department desires the services of one adjunct faculty to serve as Children's <br />Dental Care Coordinator in the Dental Service of the Orange County Health Department for the academic year <br />1999-20D0; <br />WIIIrREAS, The Health Department desires to reimburse the School of Dentistry far the adjunct faculty's <br />time commitment; <br />WIlEREA5, The School of Dentistry wishes to provide the services of one adjunct faculty to the Health <br />Department; <br />NOW, THEREFORE, in consideration of the premises and of the following mutual promises, covenants, <br />and conditions, the School of Dentistry and the Health Department agree as follows: <br />1. The Health Departtrrent will reimburse The School of Dentistry by June 30, 2000, $15,OD0.00. <br />2. The School of Dentistry will provide the services of one adjunct faculty to the Health Department <br />for the purpose of serving as the Children's Dental Care Coordinator, and the Coordinator will: <br />a. Organize the screexting assessment program in all the daycare centers and family child care <br />homes in Orange County as time permits. <br />b. Coordinate a comprehensive dental education program for parents and caregivers (i,e. <br />pediatricians, daycare staff, health department staff] of children ages 0-5. <br />c. Implement the "Tooth Time"' dental education module for preschool children which has <br />already been developed and is being utilized in other counties (Catawba, Guilford). <br />d. Coordinate a plan of treatment for incQme~ligible children. <br />3, School of Dentistry will bill The Health Department X15,000.00 by Tune 15, 2000 for the <br />salary/fringe of one adjunct faculty. <br />4. The person whose services are W be provided pursuant to the Agreement is, for all purposes an <br />employee of the University of North Carolina School of Dentistry. <br />~^ <br />
