ii) System Prevenfion <br />Participants of this group indicated that HIP, along with the City and the <br />County, should assess the problem and population initiating a citywide <br />effort with the media, police, and hospitals. A 24-hour real time <br />information line and web site needs to be developed for information <br />dissemination by the United Way Info Line, Grunwald, the media, social <br />service agencies, and the City by May 2004. An assessment team <br />involving members from jail, police, hospitals, judicial services, <br />community services, and shelters needs to estimate the population who <br />reject help and adequate case management services needs to be <br />planned by May 2004. The group also suggested that the City should <br />work with local contractors and Builder's Association to provide <br />affordable and accessible rental housing. <br />iii) Outreach <br />The main concern of this group was the lack of etluration among the <br />homeless about specific community resources. The solution suggested <br />was dissemination of information through brochures, outreach by <br />caseworkers, and by establishing aone-stop referral site. HIP should <br />offer incentives to gain participation in the continuum of care process. <br />HIP should implement and maintain the Homeless Management <br />Information System (HMIs) for universal intake and referral by 2004. <br />HIP should work with the local contractors and various agencies for <br />appropriate referrals having a Housing First case management <br />approach. <br />34 <br />
