She said in April, Alliance <br /> Health contacted the county to request authorization for use of existing Cardinal Innovation transition <br /> realignment funds held by Alliance and earmarked for Orange County priorities. She said the remaining <br /> balance of those funds, approximately $1.2 million, is planned to support the county's crisis diversion <br /> facility. <br /> Slide#3 <br /> Hope NC Proposal <br /> • Hope NC is seeking $100,000 for the remainder <br /> of their Phase 1 acquisition cost for 4 new <br /> duplexes at Weaver's Grove for their supportive <br /> housing program serving 8 adults with I/DD <br /> • The units will be leased to eligible residents with <br /> VDD earning at or under 30% AMI <br /> • The supportive services will include on-site <br /> Community Facilitators to address independent <br /> living skills, referrals to vocational and <br /> employment services and integration with the <br /> community <br /> ORANGE COUNTY <br /> NORTH CAROLINA <br /> Caitlin Fenhagen said Hope NC's proposal is part of a Phase 1 acquisition expected to close this <br /> month. She said all other funding for the purchase is secured and that the $100,000 represents the final <br /> piece needed. She said eight applications for residency have already been received, with six applicants <br /> being existing Alliance Health members and three being current Orange County residents. She said the <br /> units will be leased to eligible residents with IDD earning at or below 30%AM I.She said supportive services <br /> will include on-site community facilitators to assist with independent living skills, referrals to vocational <br /> supports, and community integration at Weaver's Grove. She said Empowerment NC will serve as the <br /> property manager. <br />
