Identify any gaps in services you intend to address by identifying <br /> and/or defining current programs/providers. <br /> • Describe the needs of the priority population that this proposal will serve and how those <br /> needs were determined(e.g.,focus groups, survey, patient engagement). <br /> Priority populations in Orange County are defined as persons with opioid us%pioid use disorder(OUD) <br /> or co-occurring substance use disorder(SUD)or mental health conditions, including: <br /> • Individuals at highest risk for overdose in Orange County, such as historically economically <br /> disadvantaged populations, individuals with multiple Adverse Childhood Experiences(ACES), <br /> Substance Use Disorder during pregnancy and in first year of Post Partum. <br /> • Children and adolescents at risk for drug use. <br /> • Individuals in treatment or recovery. <br /> • Individuals involved, or at risk of becoming involved, in the Criminal Justice System. <br /> Individuals recently released from jail or prison. <br /> NOTE:Provide citations/reference sources for any included community demographic or health status <br /> data.
