Performance Measures and Program Evaluation (18 points, page limit:not to exceed 1 <br /> page) <br /> Freedom House will use a variety of methods to evaluate project success.These include: <br /> ➢ Regular team meetings-FHRC staff will meet regularly with URD staff to review: <br /> ■ the number and flow of client referrals as they relate to service targets. Referrals will <br /> be adjusted as necessary to ensure targets are met or exceeded. <br /> ■ the number of clients successfully connected to each service and/or any additional <br /> services needed. <br /> ■ any barriers to service provision so they may quickly be resolved. <br /> ■ client satisfaction with services as evidenced by ongoing engagement in services and <br /> client satisfaction surveys administered every 6 months. <br /> ■ The resolution of social determinants of health needs such as connection to housing, <br /> medical services, harm reduction services,food assistance, and other programs <br /> necessary for sustained recovery. <br /> ➢ Formal tracking of referrals to the program, and all services received, using an internal <br /> tracking form.The internal tracking form is designed to capture all client-level data <br /> needed for quarterly and annual reports. Referrals for outpatient services that are part <br /> of the FHRC service array are made and tracked in a weekly case conference at the <br /> Freedom House outpatient clinic as well as in the electronic medical record. For services <br /> beyond those that FHRC provides,the Peer Support staff assists in reporting on <br /> successful connections each week. <br /> ➢ Client satisfaction surveys are administered to clients in the program every six months <br /> and at program discharge via the National Outcome Measures Survey (NOMS).The <br /> NOMS tool is a validated outcome measure used by the Substance Abuse and Mental <br /> Health Services Administration (SAMHSA).The NOMS measures multiple points of client <br /> satisfaction with the quality of services received and their impact on their life in <br /> recovery. <br /> ➢ FHRC includes clients in the design and implementation of program evaluation routinely. <br /> Each client signs an informed consent prior to program participation allowing the client's <br /> specific needs, services, outcomes and feedback to be shared in the weekly team <br /> staffing. Information shared in the weekly staffing is used to address the client's unique <br /> needs and circumstances as well as to address patterns of unmet needs among our <br /> clients. <br /> ➢ Additionally, Clients enrolled with Freedom House participate in the NOMS survey every <br /> six months and at program discharge. Data from the 6 months and discharge surveys are <br /> pulled monthly(as applicable)to evaluate client satisfaction with services received. <br /> Client satisfaction data is reviewed each month in Freedom House's Continuous Quality <br /> Improvement Committee and changes are made, as necessary,to increase client <br /> satisfaction with services. <br /> 9 <br />
