46 <br /> Performance Measures and Program Evaluation <br /> EI Futuro will implement a comprehensive evaluation plan to measure program impact, client <br /> outcomes, and community engagement for the SBIRT Early Intervention Program. Evaluation <br /> activities will follow evidence-based frameworks, ensuring early identification of substance use <br /> risks, timely interventions, and effective referrals to treatment. <br /> Engaging the Priority Population <br /> Latino youth, young adults, and caregivers, and others with high rates of ACEs, will be actively <br /> involved in shaping the evaluation process through advisory councils and feedback sessions. <br /> Youth and caregiver input will inform survey design, outreach approaches, and interpretation of <br /> findings, ensuring that evaluation metrics are culturally responsive and meaningful to the <br /> population served. <br /> Data Collection and Client-Level Reporting <br /> EI Futuro will collect client-level data on demographics, substance use, co-occurring mental <br /> health conditions, services received, and medications for opioid use disorder, in alignment with <br /> NCTOPPS reporting requirements. Data will be captured through EI Futuro's secure electronic <br /> health record system and SBIRT tracking forms. Monthly monitoring will ensure completeness, <br /> accuracy, and timely reporting for funders. <br /> Monitoring Metrics <br /> Key performance indicators will track program implementation, reach, and outcomes: <br /> METRIC TARGET DATA SOURCE FREQUENCY <br /> # of individuals 180+annually SBIRT Tracking Forms Monthly <br /> screened using SBIRT <br /> (Screening, Brief <br /> Intervention, and <br /> Referral to <br /> Treatment) <br /> % of screened clients 85% SBIRT Tracking Forms Quarterly <br /> receiving brief <br /> intervention <br /> % of clients identified 75% Referral Monthly <br /> as needing and documentation <br /> wanting specialized <br /> treatment, will be <br /> successfully referred <br /> to receive treatment <br /> % of workshop 85% Pre/Post Quarterly <br /> participants who <br /> report confidence in <br /> being able to identify <br /> the signs of an opioid <br /> overdose and <br /> 9 <br />
