Orange County, NC <br /> Asset Management Services <br />RFP – 367-OC5473 Orange County Solar Installation for Cedar Grove Community Center <br /> <br />5 <br /> <br /> <br />Important Information for Bidders: <br />Business Registration: Corporations, LLCs, LLPs, and foreign entities conducting business in North Carolina must <br />maintain an active registration with the NC Secretary of State in order to legally transact business with the County. <br />Safety Questionnaire <br />- The bidder must attach all supporting documentation and calculations, including certified OSHA forms. <br />- The bidder must provide the source of the EMR information and contact information of the insurer entity <br />providing the EMR. <br />- If the bidder has any of the following: (a) DART incident rate greater than its industry average, (b) and <br />EMR greater than 1.0, (c) answered “yes” to any of the OSHA Specific Questions, or (d) answered "no” to <br />any of the Safety Plan questions, the bidder shall provide the County, in its bid, a detailed written <br />explanation of its safety record and reasons why such safety history is NOT representative of its future <br />performance and what specific actions it has taken to improve its overall safety record. Failure to provide a <br />written explanation of its safety record pursuant to this paragraph may be deemed non-responsive by the <br />County. <br /> <br />-Federal Requirements <br />- The terms and conditions of this Award apply to the awarded contract, as applicable, including the <br />Intellectual Property Provisions, to all subrecipients (and subcontractors, as appropriate), as <br />required by 2 CFR 200.101, and require strict compliance therewith. Further, the terms required by <br />2 CFR 200.327 shall apply to all subrecipients (and subcontractors, as appropriate) and require strict <br />compliance therewith. <br /> <br />The bidder must return the attached forms with the bid documents. <br /> <br />Attachment A Certification Regarding Lobbying <br /> <br />Attachment D Certification Regarding Debarment, Suspension, Ineligibility, and Voluntary <br />Exclusion Lower Tier Covered Transaction <br /> <br />Attachment E Affidavit of Compliance with N.C.
