Do the residents appear neat, clean and odor free? Yes <br />2. Did residents say they receive assistance with personal care <br />activities? Ex. brushing their teeth, combing their hair, inserting <br />dentures or cleaning their eyeglasses? Yes <br />During the visit, residents were dressed and <br />ready for the day. Breakfast was over and <br />residents were napping, watching TV, or resting. <br /> <br /> <br /> <br />3. Were residents interacting with staff, other residents & visitors? <br />Yes <br />During our visit, the staff interacted with the <br />residents as needed. This was true in both the <br />assisted living and memory care sections of the <br />facility <br />5. Did residents describe their living environment as homelike? Yes We met with a resident who had recently <br />transferred from another facility in Chapel Hill, <br />and was to be discharged from this facility in a <br />few weeks, as he was there for rehab. He <br />described the difference between the 2 facilities <br />as ‘night and day’. He also wondered how the <br />other facility could pass state inspections. He <br />was glowing in his praise for the Brookdale <br />facility and their staff. <br />9. Did you notice unpleasant odors in commonly used areas? No See note below about odors and carpet <br />replacement in the entire facility. The residents <br />were complimentary about the living <br />environment. They praised the staff, the food, <br />and the activities. <br /> <br /> <br />10. Did you see items that could cause harm or be hazardous? No <br />11. Did residents feel their living areas were too noisy? No This facility is consistently quiet during our <br />quarterly visits. No one raises their voice or has <br />a TV or audio device turned to a high volume, <br />nor are the staff raising their voice while <br />conversing with residents.. <br />12. Were residents asked their preferences or opinions about the <br />activities planned for them at the facility? <br />Yes The Activity calendar displayed in the hallway <br />appeared to be well planned. Evidence of <br />resident activities such as crafts, Bingo and <br />other group activity were observed. The facility <br />halls displayed residents’ artistic talents. We <br />visited the Memory Care unit, which was a <br />restricted access unit. During our prior visits, <br />there was a dressed up Lady mannequin, which <br />was missing during this visit but we were told <br />that the mannequin was undergoing a refresh. <br />There were a variety of setups for games,
