~,.~ a~ . <br />BlueCrossBlueShield An Independent Licensee of the <br />t• • ~ of North Carolina Blue Cross and Blue Shield Association <br />PO Box 2291 <br />Durham NC 27702-2291 <br />919-489-7431 <br />MEMORANDUM <br />To: Pam Jones for Orange County Commissioners <br />Via: Mike Tapp, Orange County Fire Marshal <br />From: Rick Fahrer, Corporate Safety & Security ~~ <br />Date: May 2, 2003 <br />Subject: Request for Pyrotechnics Permit <br />On Saturday, June 7, 2003, Blue Cross and Blue Shield of North Carolina (BCBSNC) will <br />be having their Sa` annual Family Day at the Headquarters building located at 1830 Chapel <br />Hill Blvd. in Chapel Hill. This event will begin at 4:00 PM and end just after dark with a <br />short fireworks display. S & W Productions is the company employed to produce the <br />fireworks show. They also produced the identical show for our last four Family Day <br />events. They have extensive experience producing these type of small scale, low level <br />"shoots", including events at the Durham Bulls ball park, etc. <br />The Chapel Hill Fire Department will be providing a fire truck and fire personnel during <br />this event to insure adequate fire protection measures are met. The Chapel Hill Fire <br />Marshal, Deputy Chief Mellon, has reviewed this request and will make the final <br />inspection on Saturday prior to issuing the Town of Chapel Hill permit. <br />Two off-duty Chapel Hill police officers, under the direction of Capt. Everette Johnson, <br />will be employed by BCBSNC to insure adequate public safety during this event. They <br />will be assisted by members of the BCBSNC Corporate Safety & Security team as well as <br />numerous Allied Security officers. <br />Two off-duty EMTs and an ambulance will be on site during this event. BCBSNC will <br />pay Orange County EMS & Rescue for this service. <br />Copies of the operator's license, site layout, pyrotechnics inventory, and certificate of <br />insurance will be provided by Rick Timberlake of S & W Productions.
