<br /> 5 From FY 2020 through FY 2025, the Health Insurance plan has experienced increases in the
<br /> 6 frequency and severity of large claimants, overall increases in gross claims, and growth in plan
<br /> 7 enrollment.

<br /> 9 From FY 2024 to FY 2025, the cost of total claims increased by $2,803,277 or approximately
<br /> 10 19%. Of that increase, medical claims increased by $768,001 or approximately 8%, while
<br /> 11 pharmacy claims increased by over$2 million or 40%.

<br /> 13 Cost of Claims
<br /> 14 Actuarial analysis from NCHIP recommends a 12.5% rate increase to both the employee and
<br /> 15 employer share of premium equivalents to meet the cost of expected claims. If the County absorbs
<br /> 16 the entire cost of this increase and does not increase employee contributions, the County's cost
<br /> 17 would need to increase by a total of 13.7%.

<br /> 19 Pharmacy Cost Drivers
<br /> 20 One of the primary cost drivers for pharmacy claims is the increased use and high relative cost
<br /> 21 of GLP-1 drugs for weight loss.

<br /> 23 o Approximately 100 members are using GLP-1's for weight loss.
<br /> 24 o The cost of covering these medications for weight loss is approximately$1 million.
<br /> 25 o Other employers that have continued to cover the GLP-1's have also needed a
<br /> 26 "catch up" increase as employers did not anticipate the demand associated with
<br /> 27 these drugs.

<br /> 29 Medical Cost Drivers
<br /> 30 The primary cost drivers for medical claims are more varied.
<br /> 31 • The cost of cancer treatments and other cancer therapies have increased by 34%. New
<br /> 32 treatments are being discovered, approved, and put into use with unprecedented
<br /> 33 frequency. As the covered population continues to age, cancer rates are also expected to
<br /> 34 increase.
<br /> 35 • The cost of musculoskeletal outpatient surgeries has increased by 11%.
<br /> 36 • The cost of psychiatry, primary care, and specialist visits has increased by 12%. The
<br /> 37 utilization of these services by individuals covered by the Orange County plan is above
<br /> 38 the industry standard.
