Healthy Community 17 <br /> 2.1 Post Overdose Response Team ( PORT) <br /> Implementation <br /> Implement Post Overdose Response Team (PORT): <br /> • PORT contacts 90% of patients referred within 24 hours. <br /> • PORT establishes and maintains an 80% induction to transfer of care and completion of <br /> medication-assisted treatment for opioid-use disorders. <br /> In its first sixteen months of operation, PORT has not yet met the ambitious target of a 90 percent contact rate within <br /> 24 hours.The team attempts to contact patients for referral, but successful contacts are often challenging when the <br /> team does not have a patient's phone number or address. <br /> The rate of induction to transfer of care is exceeding the target of 80 percent.
