12 <br /> Exhibit"A" <br /> Scope of Services <br /> IFC Permanent Supportive Housing <br /> Term: 7/1/25-6/30/26 <br /> Amount: $150,000 <br /> Expense Description: <br /> IFC will use Orange County funding to maintain its PSH program by: <br /> • Providing rental assistance for an appropriate number of units on the caseload.The number <br /> of units will depend upon the size of households needing services and the cost of available <br /> units, especially given current Fair Market Rents (FMRs). <br /> • Ensuring compliance with HMIS and HUD record-keeping requirements. <br /> Using funds as available for critical household needs not covered by HUD funding, such as security <br /> deposits, moving and move-in costs, unit clean-up, damages, other household goods when <br /> donations are not available, and arrangements when members pass away. <br /> Concerning performance monitoring, IFC will provide upon request: <br /> • Number of total PSH clients <br /> • Number of PSH clients who stay in PSH +the ones who leave to permanent housing(goal of <br /> 80%) <br /> • Client:staff ratio(at least 15 clients for every PSH case manager) <br /> • Number of contacts ("touches") per month, per client(at least once in-person monthly, and <br /> one touch per week) <br /> • Data entered within 7 days,following NCCEH Data Quality Plan for timeliness, and meeting <br /> all target thresholds for HUD data elements <br /> • Prompt(within 2 weeks)communication regarding discharges and caseload availability <br /> • Copy of most recent program operations manual/policy <br />
