The Services to be rendered pursuant to this Agreement are as follows <br />(fully describe services to be provided): Active Threat Responder Training is an <br />emergency medical course that is designed to prepare indiThis is an emergency medical <br />course that is designed to prepare individuals to respond during active-threat incidents <br />and to casualties involving major hemorrhage or penetrating injury to chest or abdomen. <br />The training is based on TECC guidelines and is presented in the context of an active- <br />threat environment with significant focus on preparation, tactics, and reality-based <br />training techniques. <br />b. Disclaimer: Our active-bleeding training devices are breathtakingly realistic and <br />simulate actual traumatic injuries and major hemorrhage. Trainees should wear clothes <br />that can get dirty, allow movement, and be exposed to training blood. Physical activity is <br />included in the training, including kneeling, lying on the floor, and the manipulation of <br />casualties <br />c. <br />d. Training Details <br />e. • One Instructor per 10 students for hands-on training <br />f. • Duration: 0800 – 1700 (includes break times) <br />g. • Location: Onsite at client’s facility <br />h. • Instructor: Certified and Experienced Instructors with Critical Care Training <br />Expert Level Experience <br />i. • Materials Provided: <br />j. o Curriculum <br />k. o Training Equipment including Consumable Supplies <br />l. o Props <br />m. Time and Cost <br />n. Cost Notes <br />o. Course Fee for One Year of Programming 6,750 This is cost for three separate <br />trainings @ $2,250 each <br />p. <br />q. Training is a fixed cost per event with a maximum of (24) trainees per class <br />r. <br />s. OCEMS to provide training space for up to (24) w/AV gear <br />t. Courses run 8:00am - 5:00pm <br />u. <br />v. Contact melissa@medicaltrainingme.com to schedule <br />w. Set Up Our team arrives at least 45 minutes in advance to set up. We will need to <br />know what type of AV equipment is available versus our need to bring our own. We <br />prefer that your team has comfortable seating at a table as this course is video based with <br />hands-on components. <br />x. <br />y. Payment Terms: NET 30 <br />z. <br />aa.
