The work required translating federal and state mandates <br />into local, operationalized strategies, a key parallel to the current RFP scope. <br /> <br />2). Sickle Cell Disease Foundation of California (SCDF) <br />We have maintained a long-standing, multi-year partnership with the Sickle Cell Disease <br />Foundation of California (SCDF), one of the largest and oldest community -based <br />organizations serving individuals with sickle cell disease nationwide. This engagement <br />showcases THT’s ability to deliver the full range of services outlined in Orange County’s <br />RFP: from staffing assessment and workflow redesign to data system implementation and <br />financial sustainability planning. <br />Key areas of contribution included: <br />• Clinical Staffing and Operations Analysis: THT conducted a thorough review of <br />SCDF’s staffing structure across both fixed-site and mobile clinic operations. The <br />team assessed staff utilization, scheduling efficiency, and alignment of credentials <br />with roles, resulting in an optimized personnel strategy that improved service reach <br />and reduced bottlenecks in care delivery. <br />• Case Management and Data Systems Innovation : THT co-designed and <br />implemented SC CARES, a HIPAA-compliant, cloud-based case management <br />platform. This system is now nationally used across sickle cell -serving organizations <br />and enables staff to track patient encounters, document support services, analyze <br />clinical touchpoints, and report on outcomes, all in real time. <br />• Revenue Cycle and Financial Strategy: We supported SCDF in identifying <br />underutilized billing codes and opportunities to recover costs through proper <br />documentation, coding, and payer engagement.
