Key Project Personnel and Responsibilities <br />The personnel assigned to this project are carefully selected for their proven expertise and <br />relevance to the scope of services outlined in the RFP. Below are details of the team structure, <br />roles, and respective responsibilities: <br />2.2.1. Jennifer Fields, MPH <br />Project Director <br />Jennifer Fields will lead this engagement as Project Director, bringing 18+ years of <br />experience in public health administration, strategic planning, and federally funded <br />healthcare initiatives. As President & CEO of The Hills Tandem since 2014, she has <br />successfully directed regional and national projects focused on clinical efficiency, equitable <br />access, and health system transformation. <br />Education <br />• MPH, Certificate in Public Health Analysis – East Carolina University <br />• BA, Family & Consumer Science – East Carolina University <br />• MBA coursework – Florida International University <br />Relevant Experience <br />Ms. Fields has led federally funded programs and strategic initiatives with organizations <br />such as: <br />• Sickle Cell Disease Foundation of California – Co-led development of the nationally <br />adopted SC Cares Case Management System. <br />• Pacific Sickle Cell Regional Collaborative (PSCRC) – Supported multi-state care <br />coordination and provider training. <br />• Nevada Cancer Institute (CDC Grant) – Directed public health outreach and <br />compliance reporting. <br />• Imara TX & Sickle Cell Disease Association – Advanced advocacy and patient <br />engagement in chronic disease programs. <br />She currently serves as a Patient-Family Advisor for UNC Health and was appointed to the <br />NC Governor’s Council for Sickle Cell Disease (2024). <br />Role in the Project <br />As Project Director, Ms. Fields will: <br />• Oversee overall engagement strategy and deliverable quality <br />Docusign Envelope ID: 698F2B7F-27BB-45C9-9CCE-04BCCB3FB4C9
