<br /> <br />Contract-Scope of Work (7-2008) Page 2of 2 <br />C. Details of Billing process and Time Frames: <br />a. The County will reimburse Provider for costs required to provide the services <br />described in Exhibit A Section B up to the budgetary limits of the contract <br />allotment. Funds will support warehouse rent and storage expenses associated with <br />the diaper and hygiene distribution work in the county. <br />b. The Provider shall be paid in four equal installments in the amount of $5,000. The <br />first payment is contingent upon receipt of the the completed contract; the <br />remaining payments are contingent upon receipt of progres reports, which show <br />satisfactory progress twoards completion of performance measures, and an <br />accounting of expenditures and supporting documentation. <br />c. Provider will submit a progress report that includes a fiscal report and updates on <br />performance measures as outline in Exhibit A Section B. Progress report dates are: <br />July 1 - December 31; January 1 - March 31; and April 1 - June 30. Reports are <br />due within five (5) business days of the close of the reporting period. <br /> <br />D.
