FORM OF PROPOSAL <br /> <br />Attendant Booth Additions TRC PN: 670493 <br />Orange County, North Carolina AUGUST 2025 <br />BF-3 <br /> <br /> <br />2. ATTENDANT BOOTH INSTALLATION WALNUT CHURCH GROVE – This work shall consist of furnishing <br />all labor, material, equipment, tools and other items necessary and incidental to install four (4) 8’x6’ <br />Attendant Booths, install four (4) 8’x8’x8’x6” triagular concrete pads, and to install eight (8) ballards as <br />indicated in the Drawings at the Walnut Church Grove Waste & Recylcing Center. Installation shall <br />also consist of furnishing all labor, material, equipment, tools, permitting and other items necessary <br />and incidental to install electrical connections, including conduit and fasters, to the Attendant Booths. <br />Installation shall also consist of furninshing all labor, material, equipment, tools, permitting and other <br />items necessary and incidental to ensure the booths are level and installed per the manufacturer’s <br />instructions. Payment for this line item shall be made under a one -time charge and will constitute full <br />compensation for all labor, equipment, tools, and other items necessary and incidental to the <br />completion of the work. <br /> <br />3. ATTENDANT BOOTH INSTALLATION EUBANKS ROAD– This work shall consist of furnishing all labor, <br />material, equipment, tools and other items necessary and incidental to demolish and remove one (1) <br />existing Attendant Booth, and to install three (3) 8’x6’ Attendant Booths, install three (3) 8’x8’x8’x6” <br />triangular concrete pads, and to install six (6) ballards as indicated in the Drawings at the Eubanks <br />Road Waste & Recyling Center. Installation shall also consist of furnishing all labor, material, <br />equipment, tools, permitting and other items necessary and incidental to install electrical connections, <br />including conduit and fasters, to the Attendant Booths. Installation shall also consist of furninshing all <br />labor, material, equipment, tools, permitting and other items necessary and incidental to ensure the <br />booths are level and installed per the manufacturer’s instructions. Payment for this line item shall be <br />made under a one-time charge and will constitute full compensation for all labor, equipment, tools, <br />and other items necessary and incidental to the completion of the work. <br /> <br />4. ATTENDANT BOOTH INSTALLATION FERGUSON ROAD – This work shall consist of furnishing all labor, <br />material, equipment, tools and other items necessary and incidental to install one (1) 8’x6’ Attendant <br />Booth, and install three (3) ballards as indicated in the Drawings at the Ferguson Road Waste & <br />Recycling Center. Installation shall also consist of furnishing all labor, material, equipment, tools, <br />permitting and other items necessary and incidental to trech the existing gravel lot to install electrical <br />connections, including conduit and fasters, to the Attendant Booth, and the repair of the gravel lot to <br />pre-construction conditions. Installation shall also consist of furninshing all labor, material, equipment, <br />tools, permitting and other items necessary and incidental to ensure the booths are level and installed <br />per the manufacturer’s instructions. Payment for this line item shall be made under a one -time charge <br />and will constitute full compensation for all labor, equipment, tools, and other items necessary and <br />incidental to the completion of the work. <br /> <br />5. PULTRUDED FIBERGLASS ANTI-SLIP PIPE & CABLE COVER – This work shall consists of furnshing all <br />labor, material, equipment, tools and other items necessary and incidental to install Pultruded <br />Fiberglass Anti-Slip Pipe & Cable Cover over all exposed conduit on the walking surface. Payment for <br />this item shall be made on a unit price basis, as established by field measurements of final installation. <br /> <br />6. ATTENDANT BOOTH DELIVERY SCHEDULE – This time shall consist of the time necessary to order and <br />deliver the attendant booths as specified for the project from Notice to Proceed.
