The Contractor's statements must not contain sales or use taxes paid on purchases <br />of tangible personal property purchased by such Contractors for use in performing <br />the contract which does not annex to, affix to or in some manner become a part of <br />the building or structure being erected, altered or repaired for the governmental <br />units or agencies referred to in this Regulation. <br /> <br />G. The Contractor or Contractors to whom award is made on this project will be <br />required to follow the procedure outlined above. <br /> <br />H. Bidders are reminded that the total Additional 2% North Carolina Sales Use Tax <br />which was approved by Referendum in 1969 and Article 40 in 1983 and Article 42 <br />in 1986 is a part of the tax identified above. Purchases in all counties will require <br />separate certified statements for this 2% tax. <br /> <br />I. This statement shall give the vendor's name, invoice number, total amounts of <br />invoice, dates of invoices, the amount of the special 2% tax paid and the county in <br />which the purchase was made. In the event that the Contractor wishes to file a <br />combined certified statement showing the 4% tax and the 2% tax separately, this <br />will be acceptable provided he give the county in which the 2% was paid. <br /> <br />J. The certified statements of sales tax shall include all information shown on the form <br />attached at the end of this section. Comply exactly with all bidding requirements. <br />B. Show license numbers on outside of envelope. <br />C. All Bids are to be submitted on the extra unbound copy of the Bid Form. <br />D. Show all data requested on equipment and materials. <br />E. Be sure that the Bid is signed by a responsible agent of the Bidder. <br />F.
