Details of Billing process and Time Frames: <br /> <br />The provider will be paid in four equal installments in the amount of $12,500. The first <br />payment is contingent upon receipt of the agency’s performance agreement; the <br />remaining payments are contingent upon receipt of the request for reimbursement and <br />related supporting documentation. <br /> <br />Provider will submit progress reports that include a fiscal repordt and updates on <br />performance measures as outlined above. Progress report dates are: July 1 - December <br />31; January 1 - March 31, and April 1 - June 30. Once provider has satisfied its reporting <br />obligations, payment will be made within 21 days. <br /> <br />F.
