Explanation of Services to be provided and to whom: <br /> <br />Operate a successful and quality adult day health program in accordance with NC Adult <br />Day Care Standards. Maintain competitive private pay rate, as well as have options <br />available for other types of funding including VA, HCCBG, and the county respite <br />program. Hire and retain competent, skilled staff members, including RN’s and CNA’s. <br /> <br />Performance Measures Anticipated <br />Results <br />The number of participants served during FY 25-26 be at least 50 individuals. 50 <br />participants <br />The Annual Participant and Family Satisfaction Survey will inquire levels of <br />satisfaction with the following categories: personal care, physical activities, <br />cognitive activities, social activities. <br />90% <br />Individuals who serve as primary caregivers to participants will report via <br />caregiver <br />survey or inquiry, that they have maintained employment and ability to work <br />in part due to having their loved one(s) enrolled at Soltys Place. <br />100% <br />Soltys Place staff will be required to complete mandatory trainings on best <br />care practices. 100% <br /> <br />C.
