Explanation of Services to be provided and to whom: <br /> <br />Contractor will provide one full-time Family Navigator, referred to as a family mentor, <br />under the Sobriety Treatment and Recovery Team (START) program. The family mentor <br />will offer case management to families navigating the child welfare system and substance <br />use disorder treatment. This position will collaborate with child welfare staff, substance <br />use treatment providers, and other service agencies to support families with children aged <br />0-5 impacted by parental substance use. <br /> <br />The family mentor will maintain a minimum of weekly contact with each assigned <br />family. These contacts will be in accordance with the intensive case management model <br />of the START program and the individual needs of each family as determined by the <br />clinical staff at Orange County DSS and/or treatment goals for substance use services. <br />The family mentor will complete daily clinical note documenting their interactions with <br />families, case management activities, and progress toward treatment and recovery goals. <br />These notes will be available for review by the assigned child welfare supervisor and <br />administrative supervisor. <br /> <br />The contractor will provide an administrative supervisor within their organization to <br />supervise and support the family mentor. The family mentor will be considered part of <br />the care team at the Department of Social Services (DSS) and will collaborate at a <br />minimum weekly with the assigned supervisor within the Department of Social Services <br />(DSS). <br /> <br />C. Rate per unit of Service (define the unit): <br /> <br />The Orange County DSS START program can reimburse up to $71,874 toward direct and <br />indirect expenses to include salary and fringe for the Family Navigator (family mentor) <br />and the Administrative Supervisor.
