(Maximum of one page length). <br /> Section 2: Provider Qualifications <br /> Submittals must include a Provider overview and a brief description of the Provider's <br /> history. A Project organizational chart and description may be included. Key members of <br /> the Project team should be included in the organizational chart. Clearly identify the prime <br /> contractor and any sub- contractors, if relevant, and the general roles of each on the Project <br /> (Maximum of 4 pages length) <br /> Section 3: Team Member Qualifications: <br /> Detailed background information regarding personnel for at least the key team members, <br /> including: <br /> • Professional registrations and certifications, listing applicable state(s) <br /> • Office location <br /> • Position title <br /> • Length of service in current position <br /> • Role in past projects of a similar nature <br /> • A description of the current workload for identified individuals. <br /> The expectation is that the team members presented in the response will be actively <br /> engaged in the project. (Maximum of ten pages total) <br /> Section 4: An explanation of how the firm will adhere to the scope of work including <br /> a proposed work timeline that appropriately meets the goals of this project. This <br /> narrative should include an explanation of the following: <br /> • An indication of your understanding of the project. <br /> • Firm's demonstrated experience providing design services for renovation and <br /> construction projects of similar size, scope, and/or site within the past five (5) years, <br /> project team performance. <br /> • Descriptions of similar projects that the firm has completed. <br /> • Timeline shall be for all itemized projects as one project. <br /> • Any information on lawsuits filed that include the lead architectural or engineering firms <br /> within the past 5 years and the outcomes of any suits. <br /> • Information on the lead architectural and engineering firms' financial capacity to complete <br /> this project. <br /> Section 5: References <br /> A minimum of three references for similar projects, including: <br /> • The name and date of the project, <br /> • The location of the client, and <br /> • A contact name including relevant contact information. <br /> • At least one of these references should be able to describe the proposed project <br />
