NO No one mentioned sound level being a <br /> problem. <br /> F----------- <br /> 12. Does the facility accommodate smokers? YES Residents must sign in and out when <br /> Where? X Outside only they are going to the smoking area. One <br /> resident was observed leaving to smoke <br /> without noting her absence in the <br /> resident log. <br /> 13. Were residents able to reach their call bells with ease? YES We received several requests for <br /> service. We passed the request along to <br /> the nursing assistants and they <br /> responded immediately <br /> 14. Did staff answer call bells in a timely& courteous <br /> manner? YES <br /> If no, did you share this with the administrative staff? <br /> Comments/OtherObservations <br /> 15. Were residents asked their preferences or opinions YES Activities have become much more <br /> about the activities planned for them at the facility? important since the arrival of Key.. <br /> Bingo is very popular. The residents <br /> have Bingo twice a week and can win <br /> Bingo Bucks for purchases at the <br /> resident store. Prizes include shampoo, <br /> deodorant, and snacks. Donations are <br /> accepted. <br /> 16. Do residents have the opportunity to purchase <br /> personal items of their choice using their monthly YES <br /> needsfunds? <br /> Can residents access their monthly needs funds at <br /> their convenience? <br /> 17. Are residents asked their preferences about YES The residents are given a choice of <br /> meal/snack choices? several options at mealtime. Some <br /> Are they given a choice about where they prefer to residents liked the food and others felt <br /> dine? the food was adequate. <br /> 18. Do residents have privacy in making and receiving YES Two cell phones are available at the <br /> phone calls? nurses station on each hall. There are <br /> designated private areas for usage. <br /> 19. Is there evidence of community involvement from YES The Activity Calendar indicated <br /> other civic, volunteer or religious groups? community groups were participating. <br /> 20. Does the facility have a Resident's Council? YES Morris is still the President of the <br /> Family Council? No Residents Group. The attendance <br /> varies.. <br />
