EXHIBIT “B” <br />Scope of Services – FY 2025-26 <br />Outside Agency Performance Agreement <br /> <br />Agency Name: Preserving Home, Inc. (Formerly Rebuilding Together of the Triangle, Inc. <br />Program Name: Safe & Healthy Home Solutions - Orange Couty <br />Funding Award: $13,500 <br /> <br />Outline how the agency will spend Orange County’s funding award. <br />Expense Description Amount <br />Staff support for Coaltion, intake, assessment and other related tasks 11,000 <br />Fleet support for assessment and meetings assoicated with Coalition efforts 2,500 <br /> <br /> <br /> <br /> <br /> <br />Program Services <br />Outline the critical services (activities) the agency will employ to attain the Anticipated Outcomes below, by June 30, 2026. <br /> <br />- Preserving Home, Inc. (PH) will continue to help coordiant intake and eligibility verification for the Orange <br />County Home Preseveration Coalition, to ensure homeowners have an accessible way to find home repair <br />resources in the County. <br />- PH will provide a comprehensive home assessment for eligible homeowners, including a report outlining <br />what PH believes are the key home repair, weatherization and modification improvements that should be <br />prioritized for each home. These reports will be shared with the other service providers in the county to facilitate <br />project selection. <br />- PH will continue to actively participate in OCHPC meetings, selecting projects as funding permits, <br />completing repairs, and working to continue to grow the resources availabel for home preservation in Orange <br />County. <br /> <br />Orange County Strategic Plan Priority Identified in Application. If you use percentages, you must also provide the total number of participants being served by the measure. <br />Example: Priority 2, Healthy Communities <br />2.9.
