EXHIBIT “B” <br />Scope of Services – FY 2025-26 <br />Outside Agency Performance Agreement <br /> <br />Agency Name: Reentry House Plus, Inc <br />Program Name: Housing and Traiing for men in Reentry <br />Funding Award: $25,000 <br /> <br />Outline how the agency will spend Orange County’s funding award. <br />Expense Description Amount <br /> <br />Provide housing and training for men in tranisition back to society. $25,000 <br /> <br /> <br /> <br /> <br /> <br />Program Services <br />Outline the critical services (activities) the agency will employ to attain the Anticipated Outcomes below, by June 30, 2026. <br /> <br />Training with Peer Support Speciailists for the men in the residential home with the goal of providing them <br />with the skills necessary to successfully transition back into Orange County society. <br /> <br />Orange County Strategic Plan Priority Identified in Application. If you use percentages, you must also provide the total number of participants being served by the measure. <br />Example: Priority 2, Healthy Communities <br />2.9.
