Attached is my W9, the ETF form, and a copy of a canceled check. I have never carried any kind of liability insurance over my 50-year career (and counting!), as I donot create technical drawings needing to be stamped, or impart legal advice. In the few instancesover the years that a city or county has asked me for insurance prior to a workshop presentation,they have always waived it, even when my work included a site walk on undeveloped land. Hopefullythe folks at your end might be able to waive this requirement for me. I have all the TEAMS meeting invitations at hand, plus the one for Day 2 of the LUP Retreat.
