Each <br /> count is planned, coordinated, and carried out locally. <br /> OCPEH's Monthly Data Report is a new report on program outcomes that will be relayed at each <br /> monthly OCPEH Leadership Team meeting. The first monthly report was presented at the May <br /> 2025 Leadership Team meeting and included data on outcomes from the Housing Helpline, Street <br /> Outreach, Harm Reduction and Deflection (SOHRAD), SSI/SSDI Outreach,Access and Recovery <br /> (SOAR), and Housing Access Coordination.
