Visits are all-inclusive (treatment, labs, diagnostic services, education, screenings, preventive <br />care, etc) except Pharmacy - $80/visit x 12 visits per pt annually = $960 x 25 pts. = $24000 <br />MOUD medications: Burprenorphine: $20/month per pt. x 25 pts. = $500/month x 12 months = $6000 <br />$20,457 is budgeted toward 5% of a Behavioral Health Consultant salary, 2% of a second Behavioral Health <br />Consultant salary, 5% of a Project Director's salary, and 5% of the Behavioral Health Director's salary: The Behavioral <br />Health Consultants will coordinate the clinical aspects of this project, create workflows, and train staff. The Project <br />Director will create workflows, collect and analyze data, as well as coordinate and complete the billing and reporting <br />aspects of this project. The Behavioral Health Director will supervise the Behavioral Health Consultants and the <br />Project Director, as well as oversee all processes, ensuring that grant expenditure and reporting are faithfully <br />completed.
