Revised 01/24 <br />3 <br /> <br />a. Basic Services. The Services to be rendered pursuant to this Agreement are as follows <br />(fully describe services to be provided): <br />i) Provider. <br /> <br />3.a.i.1. Confidentiality. The Contractor agrees to sign the OCHD Personal Health <br />Services Division Confidentiality Agreement and Business Associate <br />Agreement, and agrees to maintain confidentiality per these Agreements. The <br />Contractor will comply with such confidentiality laws as may be applicable in <br />the performance of these agreements and acknowledges that in receiving, <br />storing, processing or otherwise dealing with any confidential information, <br />Contractor will safeguard and not further disclose the information except as <br />permitted by the Health Insurance Portability and Accountability Act of 1996, <br />Public Law 104-191, as amended. <br />3.a.i.2. Licensure. The Contractor agrees to maintain North Carolina Pharmacy <br />License and to present proof of such license. <br /> <br />ii) Orange County Health Department. <br /> <br />3.a.ii.1. Designate a nursing supervisor to provide guidance and assistance with <br />related administrative or technical aspects of providing pharmacy services at <br />Health Department sites as requested. <br />3.a.ii.2.Participate in the ongoing evaluation of the role of the pharmacist and provide <br />an annual evaluation of pharmacist’s performance of contract. <br />3.a.ii.3.Enable the pharmacist to perform all activities mandated by laws, regulations <br />and standards. <br />3.a.ii.4.Obtain required pharmacy licenses for all sites. <br />3.a.ii.5.Allow the pharmacist to participate in other related activities in the Health <br />Department as agreed to by both parties <br />3.a.ii.6.Comply with such non-discrimination laws and/or employment practices as <br />may be applicable in fulfilling this contract. <br /> <br /> <br />4. Duration of Services <br /> <br />a. Term. The term of this Agreement shall be from July 1, 2025 to June 30, 2026. <br /> <br />b. Scheduling of Services. <br />i) The Provider shall schedule and perform its activities in a timely manner. <br /> <br />ii) Should the County determine that the Provider is behind schedule, it may require <br />the Provider to expedite and accelerate its efforts, including providing additional <br />resources and working overtime, as necessary, to perform its services in <br />accordance with the approved project schedule at no additional cost to the <br />County. <br /> <br />iii) The Commencement Date for the Provider's Basic Services shall be July 1, 2025. <br /> <br />5.
