<br /> <br /> www.callmc.com <br /> <br />Mobile Communications America, Inc. <br />Confidential and Proprietary <br />Not to be copied or distributed without the express written consent of Mobile Communications America, Inc. <br /> <br /> <br />Equipment Maintenance Agreement by and between <br />Mobile Communications America”MCA” <br />& Customer Name <br /> <br />This Equipment Maintenance Agreement, dated as of Month Day, Year (the “Effective Date”), is entered <br />into by and between Mobile Communications America “MCA”, having its principal place of business <br />at 100 Dunbar St. Suite 304 Spartanburg, SC 29306, (hereinafter referred to as “MCA”), and Customer <br />Name, having its principal place of business at Street Address, City, State Zip Code (hereinafter <br />collectively referred to as the “Customer”). <br /> <br />1. Covered Equipment <br /> <br />The Equipment for which maintenance is offered under this Agreement is described in detail in Exhibit <br />“A” hereto, as amended from time to time (the “Equipment.”) <br /> <br />2. Included Services <br /> <br />MCA shall provide the following maintenance services on the Equipment under this Agreement, located <br />at: <br />1. Location 1 <br />2. Location 2 <br />3. Location 3 <br /> <br />2.1 Annual Maintenance <br /> <br />2.1.1 Check all equipment for any operational anomalies. <br />2.1.2 Ensure that any potentially System degrading foreign substances such as oil, dust, or moisture are <br />removed from the equipment. <br />2.1.3 Measure the composite RF input power to the System/Systems. <br />2.1.4 Measure the RF power per carrier into the System/Systems. <br />2.1.5 Take samplings of RF levels at the site to verify that performance is consistent with as-built <br />documentation. <br /> <br />2.2 Public Safety Recertification <br /> <br />2.2.1 Check BDA and BBU for existing alarms and record findings. <br />2.2.2 Upgrade BDA and BBU, if applicable, to latest firmware. <br />2.2.3 Test BBU by checking individual battery cells and recording measurements. If required by local <br />code, power will be removed from the BBU for 1-hr to ensure that the BDA remains operational. <br />2.2.4 Record donor signal measurements by connecting spectrum analyzer to the donor line and <br />measuring RF level of the local control channel. If no control channel exists, level from an active <br />channel will be recorded. <br />2.2.5 Record system gain, donor isolation and azimuth. <br />2.2.6 Complete grid test of building as required by local code. <br />2.2.7 Capture photos and screenshots from head end equipment. <br />2.2.8 Complete recertification report and submit as required by local code. <br />Docusign Envelope ID: E2A04EAA-C6A8-4D8C-A3D2-7BFA20FBDAF0
