29 <br /> <br /> <br /> <br /> <br /> <br /> <br />Worker’s Compensation <br />We provide a comprehensive workers’ compensation insurance program. The program covers most accidental <br />injury or illness sustained in the course of employment that requires medical, surgical or hospital treatment. <br />Neither the insurance carrier nor we will be liable for the payment or workers’ compensation benefits for <br />injuries that occur during employees’ voluntary participation in any off-duty activities sponsored by our <br />company. <br /> <br />Reporting an Accident. If you sustain a work-related injury or illness (no matter how minor an on-the-job <br />injury may appear), you must inform your supervisor immediately. Report the incident/accident by following <br />this procedure: <br /> <br />1. If the incident/accident is Life Threatening, dial 911 immediately and obtain emergency care. <br />2. Complete a “Claims Reporting Form”. <br />3. Contact Office Manager <br />4. All injuries must have a Drug and Alcohol Test or Workmen’s Compensation will not cover your <br />doctor’s bills. <br /> <br />Your supervisor may provide you with other “medical reporting forms” that either you or your doctor need to <br />complete each time you go to the doctor. If so, the forms need to be returned to your supervisor. You may be <br />required to seek care through a panel of pre-approved doctors or your claim may be denied. <br /> <br /> <br />Safety <br />All employees must perform their duties in a manner that insures safety for themselves, their co-workers, and <br />the company. Each employee must practice safety awareness by thinking defensively, anticipating unsafe <br />situations and reporting unsafe conditions immediately. <br /> <br />Please observe the following precautions: <br /> <br />1. Notify your manager of any emergency situation. If you are injured or become sick at work, no matter <br />how slight, you must inform your worksite supervisor immediately. <br />2. Use, adjust, and repair machinery and equipment only if you are trained and qualified. <br />3. Use the correct equipment for all duties and avoid shortcuts or substitutes that can lead to injury. <br />4. Seek help when lifting, pushing or otherwise moving heavy objects. <br />5. Wipe up all spills immediately and keep floors clean and dry. <br />6. Store and stack equipment, files, boxes, etc. in a safe manner and do not block exits or safety <br />equipment. <br />7. Do not climb shelves or counters that are not designed for such use; instead, use approved step ladders <br />8. Understand your job fully and follow instructions. If you are not sure of the safe procedure, ask your <br />supervisor. <br />9. Know the locations, contents, and the use of first aid and fire fighting equipment <br />10. Report any unsafe conditions or fire hazards to management immediately <br />11. Wear and utilize all approved and required safety equipment/clothing. <br />12.
