DOA-73.3 <br />(Itev. 2/04) <br />Home and Commnnity Care Block Grant for Older Adults <br />County Funding Plan <br />July 1, 2004 through Jnne .30, 2005 <br />Methodology to Address Service Needs ofLow-hrcome Minority Elderly <br />(Older Americans Act, Section .306(a)(4)(A)(ii) <br />Community Service Provider ~-~r~-r°=~ L'O~ ~0~~ r~3~~~J~c~~--J <br />County~Qll~--;~ <br />The Older Americans Ack requires that the service provider attempt to provide services to <br />low=income minority individuals in accordance to their need for aging services. The <br />community service provider shall specify how the service needs oflow-income minority <br />elderly and rural elderly will be met through the services identified on the Provider <br />Services Summary (DOA-732).. This narrative shall address ouheach and service <br />delivery methodologies that will ensure that this target population is adequately served <br />and conform with specific objectives established by the Area Agency on Aging, far <br />providing services to low income minority individuals.
