The <br />community service provider shall specify how the service needs oflow-income minority <br />elderly and rural elderly will be met through the services identified on the Provider <br />Services Summary (DOA-732).. This narrative shall address ouheach and service <br />delivery methodologies that will ensure that this target population is adequately served <br />and conform with specific objectives established by the Area Agency on Aging, far <br />providing services to low income minority individuals. Additional pages may be used as <br />necessary. <br />
