This proposal expires 8/1/2025. <br />Contact: Matthew McKerr, IT Systems Analyst, mmckerr@orangecountync.gov - 919-245-2284 <br /> <br />Purpose: This is a Renewal Addendum for the upcoming 2025-2026 subscription year. <br /> <br />Orange County Health Department intends to continue using Patagonia Health Medication Inventory/ <br />Pharmacy Solution for the 2025-2026 year. This excludes the use of any billing or electronic prescription <br />functionality. <br /> <br />Agreement Term 1 Year: Start Date 7/1/2025, End Date 6/30/2026 <br />● Users Include: 30 licensed users. <br />● Excludes use of and support for all other applications & interfaces, including billing and <br />e-prescription applications. <br /> <br />Functionality and Costs: <br /> <br />Description Annual Subscription <br />Functionality: Pharmacy Solution for the Health Department includes Surescript <br />certified electronic prescription solution and support for up to thirty (30) users. <br />$17,234.64 <br /> <br />Terms: <br />● Subscription Fees will be invoiced annually, via ACH electronic funds transfer, and will increase <br />each anniversary on July 1st by 4%. <br />● This Agreement ’s minimum term is for one year. After the initial one-year term, the agreement <br />will automatically renew for the next year unless written notice is received at least 90 days <br />prior to the expiration of the current term to discuss new terms or to terminate the <br />agreement. <br />● Reactivation post-cancellation will incur an initiation fee. <br />● User Licenses can be added for $35.00 per user at any time during the active subscription. <br />● A decrease in user licenses cannot occur more than once in six months. <br /> <br />Page 1 of 2 <br />Confidential.
