38 <br /> • Elderly: Increase the availability of accessible and age-friendly housing by rehabilitating <br /> existing units to include universal design features and constructing new affordable housing <br /> with built-in accessibility accommodations. <br /> • Frail Elderly: Expand housing options that integrate accessibility features and on-site <br /> supportive services, such as assisted living facilities, home health services, and <br /> transportation assistance,to enable seniors to age in place safely. <br /> • People with Disabilities: Prioritize funding for home modifications and rehabilitation to <br /> improve accessibility, increase enforcement of reasonable accommodation policies in <br /> rental housing, and promote the new construction of affordable,fully accessible units that <br /> meet the diverse needs of individuals with disabilities. <br /> • People with Substance Use Disorder: Expand permanent supportive housing and <br /> transitional housing options that integrate case management, recovery services, and peer <br /> support programs to promote long-term housing stability and recovery. <br /> • People with HIV/AIDS and Their Families: Increase the supply of permanent supportive <br /> housing with wraparound services, including medical care, mental health counseling, and <br /> case management,to ensure stable housing and improved health outcomes. <br /> • People Residing in Public Housing: Enhance supportive services to promote economic self- <br /> sufficiency, provide rental assistance and relocation support for those seeking affordable <br /> private-market housing, and develop new high-quality, affordable rental housing to address <br /> ongoing demand. <br /> Describe programs for ensuring that persons returning from mental and physical health <br /> institutions receive appropriate supportive housing <br /> The Orange County Partnership to End Homelessness oversees the Continuum of Care for Orange <br /> County, ensuring a coordinated and strategic approach to addressing homelessness.As part of its <br /> efforts,the partnership has implemented a robust Coordinated Entry System designed to assess <br /> the unique needs of individuals transitioning from mental health and physical care institutions.This <br /> system prioritizes these individuals for Permanent Supportive Housing, ensuring they receive the <br /> necessary resources and support to achieve stability and long-term housing security. <br /> Specify the activities that the jurisdiction plans to undertake during the next year to address <br /> the housing and supportive services needs identified in accordance with 91.215(e)with <br /> respect to persons who are not homeless but have other special needs.
