Artwork to be supplied by customer in a HIGH-RESOLUTION <br />PDF or EPS file. If artwork cannot be supplied as requested, we can provide artwork set up for an <br />additional $300.00. Please sign and date both pages of this quote and return with a Municipal <br />Purchase Order or a deposit check for 50% of the total amount made payable to: “Armor Tuff, LLC”. <br />Balance due on the day of completion. <br /> <br />Please note: Items in this section may or may not apply to your station. Existing surface other than <br />bare concrete may have to be removed. These surfaces include epoxy, paint, quartz/resin floors, <br />epoxy floors, tile, mastic, sealers, oil and grease. There may be an additional charge for providing <br />floor preparation services. Pricing based on thickness of existing floor surface to be removed. If <br />necessary, travel and lodging expenses are to be paid for by customer with installer’s receipts if <br />necessary. Labor quoted with prevailing wage rates.
