54 <br /> F. Performance Measures and Program Evaluation (18 points, page limit:not to exceed 1 page) <br /> This project will be evaluated via quantitative and qualitative data. Our quarterly goals are to <br /> serve nine Orange County residents with MOUD services at no cost to the patient, including four <br /> provider visits and four Behavioral Health Consultant visits per quarter, all essential lab tests, <br /> and monthly medication (amount/exact medication and labs will depend on patient need). <br /> Quantitative data will be captured via our Electronic Medical Records and will include the <br /> following: <br /> - Number of patients who receive MOUD services <br /> - Number of appointments/visits attended <br /> - Number and type of labs provided <br /> - Amount and type of medication provided <br /> - Patient demographics <br /> - Patient diagnoses <br /> Qualitative data will be captured via patient survey and will include: <br /> - Patient satisfaction level <br /> - Any comments or suggestions <br /> Evaluation will be performed by Admin staff at the midpoint and conclusion of the 12-month <br /> program. <br /> 10 <br />
