<br />Phone: (336)449-1144 PO Box 10, Whitsett NC 27377 Fax: (336)449-1151 <br /> www.SASSERCOMPANIES.com <br /> <br /> <br />Remediation Services: <br />• Construct containment with 6 mil poly to prevent dust/particulates from exiting work area and <br />affecting non-affected areas. <br />o All cracks, crevasse, windows or any entry points will be sealed off with 6 mil poly and <br />tape. <br />o Containment will be constructed around the shelves after performing the cleaning <br />process. <br />• Installed negative pressure on containments utilizing engineering controls (Air Scrubbers) <br />o Utilize exterior door for negative pressure. <br />• Clean all rooms based on Hygienist scope. Hepa sandwich (Hepa vacuum/Damp Wipe/Hepa <br />vacuum again) <br />o Walls/Floors/Furniture/Evidence boxes will be cleaned <br /> Walls will be cleaned up to 6’ from floor (Hygienist protocol) <br />• ULV Fogging with Benefect antimicrobial will occur every day before the team departs for the day <br />to control dust/particulates. <br />o Air scrubbers will be turned off during the fogging process. <br />• Once the cleaning stage has been completed the team will detach air scrubbers from negative <br />and place them under neutral pressure and change pre and pleated filters. Air scrubbers will run <br />on neutral for at least 24 hours before PRV testing takes place. <br />• All personnel will wear the appropriate PPE during the remediation process. <br />• Debris Disposal <br /> <br /> <br /> <br />Project Exclusions: <br />• Replacement of any boxes/materials that are required to be discarded. <br /> <br />Expected timeline: <br />The remediation team will be onsite for 7 days to complete the following: <br />• Remediation (Cleaning) – 4 Days <br />• 24 Hour scrub time – 1 Day <br />• PRV Testing – 1 Day <br />• Removal of containment – 1 Day <br />This proposal is to complete this project, as detailed above, for a not-to-exceed price of: <br />Not-to-Exceed $24,500.00 <br /> <br /> <br />This proposal is valid for acceptance for a period of thirty (30) days, after thirty (30) days Sasser <br />Restoration, LLC reserves the right to reevaluate the proposal to reflect current market conditions. <br />Docusign Envelope ID: 5BB51622-E34C-499B-8450-449F7B09BA04
