The funding from this grant will be used to support the FIT <br /> Program Lead Community Health Worker. This budget amendment provides for the receipt <br /> of $15,457 from the grant award to be spent on personnel costs. <br /> 9. The Health Department has received $1,935 in state funding for Child Fatality Reporting. <br /> These funds will be used to prepare and begin using the NFR-CRS web based <br /> standardized case reporting tool. Funds were appropriated by the General Assembly to <br /> support implementation of the changes authorized by North Carolina Session Law (NCSL) <br /> 2023-134 to restructure child death reviews by local teams and to offset the costs <br /> associated with local team participation in NFR-CRS. This budget amendment provides for <br /> the receipt of $1,935 from the grant award to be spent on contract services. <br /> 10.The Health Department has received $12,335 in federal pass-through funding for <br /> Investment in Performance Management. These funds will be used towards the purchase <br /> of Charity Tracker software. This database software will be used by the Family Success <br /> Alliance (FSA) for case management to track visits and activities for each client served. <br /> Some of the biggest barriers that FSA looks at are clients' transportation finances, do they <br /> have access to insurance, to the clinics, and if finances are an issue due to employment, <br /> FSA can help individuals find jobs through their case management. The software and <br /> underlying database will then enable the Navigators to report their activities to the state <br /> more easily and accurately. This budget amendment provides for the receipt of $12,335 <br /> from the grant award to be spent on equipment costs. <br /> ALIGNMENT WITH STRATEGIC PLAN: This item supports: <br /> • GOAL 2: HEALTHY COMMUNITY <br /> OBJECTIVE 1. Expand access to quality, affordable healthcare services. (e.g., Crisis <br /> Diversion facility, Medicaid expansion, crisis response, healthy living campaign). <br /> OBJECTIVE 3. Provide social safety net programming and the resources needed for <br /> our most vulnerable community members (e.g., veterans, unhoused people, foster <br /> children, older adults, etc.). <br /> • GOAL 4: MULTI-MODAL TRANSPORTATION <br /> OBJECTIVE 2. Increase community awareness of all modes of transportation including <br /> transit, bike and pedestrian, vehicle, and all other modes. <br /> Department on Aging <br /> 11.The Orange County Department on Aging has received notification of additional Home and <br /> Community Care Block Grant funds, distributed by the Central Pines Council of <br /> Governments, in the amount of $31,280.
