5 <br /> <br /> <br />6.0 Adjustments to Incident Intake Form <br /> <br />ClearRisk will be making the following modifications to Orange County’s existing <br />incident intake form: <br /> <br />● Addition of a pick-list question “Did this incident take place on county <br />property?” <br />- If answered “Yes”, a “Lookup” field will show up allowing form <br />submitters to select from the properties/buildings in the ClearRisk <br />system (to be uploaded as part of the Phase 2 project). <br />- If answered “No”, a field populates to allow form submitters to <br />enter an address manually. <br /> <br />7.0 Training Sessions & Self-Guided Training Materials <br /> <br />7.1 Live Training sessions <br />ClearRisk will be providing five (5) online training sessions that will be facilitated <br />via Zoom. These sessions will be recorded and sent to the ORANGE COUNTY <br />and follow a standard agenda meaning topics will be covered in a rigid order <br />across training sessions. <br /> <br />8.0 Project Controls <br /> <br />8.1 Inspection & Acceptance <br />ORANGE COUNTY shall inspect all Deliverables that ClearRisk develops under <br />this SOW. ClearRisk shall inform ORANGE COUNTY of the due dates for all <br />inspections and provide materials of the Deliverable subject to inspection at <br />least one week before the inspection due date unless otherwise specified. <br />ClearRisk will provide deliverable acceptance forms that once returned will <br />signify deliverable sign-off/completion. The CUSTOMER completing its <br />inspection and acceptance by the agreed due dates is critical for the overall <br />project timeline.
