AmericanFacility <br />SERVICES <br /> <br /> <br /> <br /> A Deficiency Report is initiated to correct any task where the performance <br />level has diminished below previous levels. <br /> A Corrective Action Request is initiated to correct any task that has been <br />rated less than "satisfactory;" <br /> Inspection Reports will be submitted to the Contract Manager, our Corporate <br />Office, and the Client. <br /> Routine monitoring activities, such as our continuous inspections, also <br />significantly reduce the potential for future situations by signaling the need for <br />timely corrective actions. <br /> A combination of these activities, carefully supplemented by the previously <br />identified methods, will be applied to prevent deficiencies and, where <br />necessary, attain the earliest possible correction. <br /> <br /> <br /> <br />Figure 2 - Corrective Action Cycle <br /> <br />Information to QCM and PM <br />for preventative measures <br />and/or policy changes <br />Inspection as Required <br />Problem Identification Deficiency Report <br />Reinspection <br />Trend Analysis <br />Deficiency Corrected <br /> <br /> <br />Corrective Actions, Long and Short-Term <br /> <br />If a deficiency does occur, AFS uses a two-tier strategy to address the problem. First, in the <br />short term, the Site Supervisor will generate a Deficiency Report and initiate corrective actions <br />immediately to eliminate the situation. After correction, the work is re-inspected to ensure that <br />our customer’s and corporate quality control standards are met. The Site Supervisor will then <br />complete the report detailing what corrective actions were taken.
